How CoRE Care can help

Dementia Guidance Consultation
If you or your family member has been diagnosed with dementia you may have many questions including:
-
what to expect
-
what to do next
-
what services can support you
-
how to access support services
-
self-care and caring for your loved one
-
coping with challenges including behaviour changes
-
how to support your loved one to stay at home

A Dementia Guidance Consultation is a person-centred, professional consultation in the comfort of your own home.

CoRE Care support starts with a free initial telephone call to discuss your needs and how Dementia Guidance may help.
An in-person Dementia Guidance Consultation can then be arranged to help identify your needs and provide professional and compassionate guidance including:
-
understanding dementia
-
care planning
-
care navigation
-
individualised behaviour support if needed
In-person services may be funded by HCP or NDIS packages.
